Submit Your Award
So you have completed your Award level… here are the next steps:
Get all of your Award material together. Make sure that you have included an application form and checklist for your appropriate level. Please GO OVER THE CHECKLIST to make sure that you have completed everything.
If you are missing an application form or check list, you can download one from the link below. Print it off, fill it out, and send it in.

-Silver Application Form & Checklist
-Gold Application Form & Checklist
Please send in your completed Awards to the Ontario Office to be reviewed by the Award Assessment Committee. This committee meets about once a month. Please understand that it will take about 6 weeks once you have submitted your award before you get it returned to you.
If there are any questions or concerns with your award, we will contact you personally.
All of the Award Material you submit will be returned to you in the same condition it was sent it, expect for the Award Application. This goes on file in the Ontario Office.
